- Proof of Active Authority (MC Certificate)
- Certificate of Insurance (Minimum Requirements: $1M Auto Liability, $100K Cargo)
- Voided Check or Bank Letter (for ACH payments)
- Copy of CDL and Equipment Registration (if requested)
Payment Terms
